JENIS-JENIS NARKOBA

Narkoba merupakan singkatan dari Narkotika dan Obat/Bahan berbahaya yang telah populer beredar dimasyarakat perkotaan maupun di pedesaan, termasuk bagi aparat hukum. Sebenarnya dahulu kala masyarakat juga mengenal istilah madat sebagai sebutan untuk candu atau opium, suatu golongan narkotika yang berasal dari getah kuncup bunga tanaman Poppy yang banyak tumbuh di sekitar Thailand, Myanmar dan Laos (The Golden Triangle) maupun di Pakistan dan Afganistan.

Selain Narkoba, istilah lain yang diperkenalkan khususnya oleh Departemen Kesehatan RI adalah NAPZA yaitu singkatan dari Narkotika, Pasikotropika dan Zat adiktif lainnya. Semua istilah ini sebenarnya mengacu pada sekelompok zat yang umumnya mempunyai risiko yang oleh masyarakat disebut berbahaya yaitu kecanduan (adiksi).
Narkoba atau NAPZA merupakan bahan/zat yang bila masuk ke dalam tubuh akan mempengaruhi tubuh terutama susunan syaraf pusat/otak sehingga bilamana disalahgunakan akan menyebabkan gangguan fisik, psikis/jiwa dan fungsi sosial. Karena itu Pemerintah memberlakukan Undang-Undang untuk penyalahgunaan narkoba yaitu UU No.5 tahun 1997 tentang Psikotropika dan UU No.22 tahun 1997 tentang Narkotika.
Golongan Psikotropika adalah zat atau obat baik alami maupun sintetis namun bukan Narkotika yang berkhasiat aktif terhadap kejiwaan (psikoaktif) melalui pengaruhnya pada susunan syaraf pusat sehingga menimbulkan perubahaan tertentu pada aktivitas mental dan perilaku.
Narkotika adalah zat atau obat yang berasal dari tanaman atau bukan tanaman baik sintetis maupun semisintetis yang akan menyebabkan perubahan kesadaran, mengurangi sampai menghilangkan rasa sakit dan dapat menimbulkan ketergantungan (adiksi).
Jenis Narkotika yang sering disalahgunakan adalah morfin, heroin (putauw), petidin, termasuk ganja atau kanabis, mariyuana, hashis dan kokain.
Sedangkan jenis Psikotropika yang sering disalahgunakan adalah amfetamin, ekstasi, shabu, obat penenang seperti mogadon, rohypnol, dumolid, lexotan, pil koplo, BK, termasuk LSD, Mushroom.
Zat adiktif lainnya disini adalah bahan/zat bukan Narkotika & Psikotropika seperti alkohol/etanol atau metanol, tembakau, gas yang dihirup (inhalansia) maupun zat pelarut (solven).
Sering kali pemakaian rokok dan alkohol terutama pada kelompok remaja (usia 14-20 tahun) harus diwaspadai orangtua karena umumnya pemakaian kedua zat tersebut cenderung menjadi pintu masuk penyalahgunaan Narkoba lain yang lebih berbahaya (Putauw).

OPIAT atau Opium (candu)

Merupakan golongan Narkotika alami yang sering digunakan dengan cara dihisap (inhalasi).

• Menimbulkan rasa kesibukan (rushing sensation)
• Menimbulkan semangat
• Merasa waktu berjalan lambat.
• Pusing, kehilangan keseimbangan/mabuk.
• Merasa rangsang birahi meningkat (hambatan seksual hilang).
• Timbul masalah kulit di sekitar mulut dan hidung.

MORFIN
Merupakan zat aktif (narkotika) yang diperoleh dari candu melalui pengolahan secara kimia. Umumnya candu mengandung 10% morfin. Cara pemakaiannya disuntik di bawah kulit, ke dalam otot atau pembuluh darah (intravena)

• Menimbulkan euforia.
• Mual, muntah, sulit buang hajat besar (konstipasi).
• Kebingungan (konfusi).
• Berkeringat.
• Dapat menyebabkan pingsan, jantung berdebar-debar.
• Gelisah dan perubahan suasana hati.
• Mulut kering dan warna muka berubah. 

HEROIN atau Putaw
Merupakan golongan narkotika semisintetis yang dihasilkan atas pengolahan morfin secara kimiawi melalui 4 tahapan sehingga diperoleh heroin paling murni berkadar 80% hingga 99%. Heroin murni berbentuk bubuk putih sedangkan heroin tidak murni berwarna putih keabuan (street heroin). Zat ini sangat mudah menembus otak sehingga bereaksi lebih kuat dari pada morfin itu sendiri. Umumnya digunakan dengan cara disuntik atau dihisap. Timbul rasa kesibukan yang sangat cepat/rushing sensastion (± 30-60 detik) diikuti rasa menyenangkan seperti mimpi yang penuh kedamaian dan kepuasan atau ketenangan hati (euforia). Ingin selalu menyendiri untuk menikmatinya.

• Denyut nadi melambat.
• Tekanan darah menurun.
• Otot-otot menjadi lemas/relaks.
• Diafragma mata (pupil) mengecil (pin point).
• Mengurangi bahkan menghilangkan kepercayaan diri.
• Membentuk dunia sendiri (dissosial) : tidak bersahabat.
• Penyimpangan perilaku : berbohong, menipu, mencuri, kriminal.
• Ketergantungan dapat terjadi dalam beberapa hari.
• Efek samping timbul kesulitan dorongan seksual, kesulitan membuang hajat besar, jantung berdebar-debar, kemerahan dan gatal di sekitar hidung, timbul gangguan kebiasaan tidur.

Jika sudah toleransi, semakin mudah depresi dan marah sedangkan efek euforia semakin ringan atau singkat

GANJA atau kanabis
Berasal dari tanaman kanabis sativa dan kanabis indica. Pada tanaman ini terkandung 3 zat utama yaitu tetrahidrokanabinol, kanabinol dan kanabidiol. Cara penggunaannya dihisap dengan cara dipadatkan menyerupai rokok atau dengan menggunakan pipa rokok.

• Denyut jantung atau nadi lebih cepat.
• Mulut dan tenggorokan kering.
• Merasa lebih santai, banyak bicara dan bergembira.
• Sulit mengingat sesuatu kejadian.
• Kesulitan kinerja yang membutuhkan konsentrasi, reaksi yang cepat dan koordinasi.
• Kadang-kadang menjadi agresif bahkan kekerasan.
• Bilamana pemakaian dihentikan dapat diikuti dengan sakit kepala, mual yang berkepanjangan, rasa letih/capek.
• Gangguan kebiasaan tidur.
• Sensitif dan gelisah.
• Berkeringat.
• Berfantasi.
• Selera makan bertambah.

LSD atau lysergic acid atau acid, trips, tabs
Termasuk sebagai golongan halusinogen (membuat khayalan) yang biasa diperoleh dalam bentuk kertas berukuran kotak kecil sebesar ¼ perangko dalam banyak warna dan gambar. Ada juga yang berbentuk pil atau kapsul. Cara menggunakannya dengan meletakkan LSD pada permukaan lidah dan bereaksi setelah 30-60 menit kemudian dan berakhir setelah 8-12 jam.

• Timbul rasa yang disebut Tripping yaitu seperti halusinasi tempat, warna dan waktu.
• Biasanya halusinasi ini digabung menjadi satu hingga timbul obsesi terhadap yang dirasakan dan ingin hanyut di dalamnya.
• Menjadi sangat indah atau bahkan menyeramkan dan lama kelamaan membuat perasaan khawatir yang berlebihan (paranoid).
• Denyut jantung dan tekanan darah meningkat.
• Diafragma mata melebar dan demam.
• Disorientasi.
• Depresi.
• Pusing
• Panik dan rasa takut berlebihan.
• Flashback (mengingat masa lalu) selama beberapa minggu atau bulan kemudian.
• Gangguan persepsi seperti merasa kurus atau kehilangan berat badan. 

KOKAIN
Mempunyai 2 bentuk yakni bentuk asam (kokain hidroklorida) dan bentuk basa (free base). Kokain asam berupa kristal putih, rasa sedikit pahit dan lebih mudah larut dibanding bentuk basa bebas yang tidak berbau dan rasanya pahit. Nama jalanan kadang disebut koka, coke, happy dust, snow, charlie, srepet, salju, putih. Disalahgunakan dengan cara menghirup yaitu membagi setumpuk kokain menjadi beberapa bagian berbaris lurus di atas permukaan kaca dan benda yang mempunyai permukaan datar. Kemudian dihirup dengan menggunakan penyedot atau gulungan kertas. Cara lain adalah dibakar bersama tembakau yang sering disebut cocopuff. Menghirup kokain berisiko luka pada sekitar lubang hidung bagian dalam.

• Menimbulkan keriangan, kegembiraan yang berlebihan (ecstasy).
• Hasutan (agitasi), kegelisahan, kewaspadaan dan dorongan seks.
• Penggunaan jangka panjang mengurangi berat badan.
• Timbul masalah kulit.
• Kejang-kejang, kesulitan bernafas.
• Sering mengeluarkan dahak atau lendir.
• Merokok kokain merusak paru (emfisema).
• Memperlambat pencernaan dan menutupi selera makan.
• Paranoid.
• Merasa seperti ada kutu yang merambat di atas kulit (cocaine bugs).
• Gangguan penglihatan (snow light).
• Kebingungan (konfusi).
• Bicara seperti menelan (slurred speech). 

AMFETAMIN
Nama generik/turunan amfetamin adalah D-pseudo epinefrin yang pertama kali disintesis pada tahun 1887 dan dipasarkan tahun 1932 sebagai pengurang sumbatan hidung (dekongestan). Berupa bubuk warna putih dan keabu-abuan. Ada 2 jenis amfetamin yaitu MDMA (metil dioksi metamfetamin) dikenal dengan nama ectacy. Nama lain fantacy pils, inex. Metamfetamin bekerja lebih lama dibanding MDMA (dapat mencapai 12 jam) dan efek halusinasinya lebih kuat. Nama lainnya shabu, SS, ice. Cara penggunaan dalam bentuk pil diminum. Dalam bentuk kristal dibakar dengan menggunakan kertas alumunium foil dan asapnya dihisap melalui hidung, atau dibakar dengan memakai botol kaca yang dirancang khusus (bong). Dalam bentuk kristal yang dilarutkan dapat juga melalui suntikan ke dalam pembuluh darah (intravena).

• Jantung terasa sangat berdebar-debar (heart thumps).
• Suhu badan naik/demam.
• Tidak bisa tidur.
• Merasa sangat bergembira (euforia).
• Menimbulkan hasutan (agitasi).
• Banyak bicara (talkativeness).
• Menjadi lebih berani/agresif.
• Kehilangan nafsu makan.
• Mulut kering dan merasa haus.
• Berkeringat.
• Tekanan darah meningkat.
• Mual dan merasa sakit.
• Sakit kepala, pusing, tremor/gemetar.
• Timbul rasa letih, takut dan depresi dalam beberapa hari.
• Gigi rapuh, gusi menyusut karena kekurangan kalsium.

SEDATIF-HIPNOTIK (Benzodiazepin/BDZ)
Sedatif (obat penenang) dan hipnotikum (obat tidur). Nama jalanan BDZ antara lain BK, Lexo, MG, Rohip, Dum. Cara pemakaian BDZ dapat diminum, disuntik intravena, dan melalui dubur. Ada yang minum BDZ mencapai lebih dari 30 tablet sekaligus. Dosis mematikan/letal tidak diketahui dengan pasti. Bila BDZ dicampur dengan zat lain seperti alkohol, putauw bisa berakibat fatal karena menekan sistem pusat pernafasan. Umumnya dokter memberi obat ini untuk mengatasi kecemasan atau panik serta pengaruh tidur sebagai efek utamanya, misalnya aprazolam/Xanax/Alviz.

• Akan mengurangi pengendalian diri dan pengambilan keputusan.
• Menjadi sangat acuh atau tidak peduli dan bila disuntik akan menambah risiko terinfeksi HIV/AIDS dan hepatitis B & C akibat pemakaian jarum bersama.

Obat tidur/hipnotikum terutama golongan barbiturat dapat disalahgunakan misalnya seconal.

• Terjadi gangguan konsentrasi dan keterampilan yang berkepanjangan.
• Menghilangkan kekhawatiran dan ketegangan (tension).
• Perilaku aneh atau menunjukkan tanda kebingungan proses berpikir.
• Nampak bahagia dan santai.
• Bicara seperti sambil menelan (slurred speech).
• Jalan sempoyongan.
• Tidak bisa memberi pendapat dengan baik.

ALKOHOL
Merupakan suatu zat yang paling sering disalahgunakan manusia. Alkohol diperoleh atas peragian/fermentasi madu, gula, sari buah atau umbi-umbian. Dari peragian tersebut dapat diperoleh alkohol sampai 15% tetapi dengan proses penyulingan (destilasi) dapat dihasilkan kadar alkohol yang lebih tinggi bahkan mencapai 100%. Kadar alkohol dalam darah maksimum dicapai 30-90 menit. Setelah diserap, alkohol/etanol disebarluaskan ke suluruh jaringan dan cairan tubuh. Dengan peningkatan kadar alkohol dalam darah orang akan menjadi euforia, namun dengan penurunannya orang tersebut menjadi depresi. Dikenal 3 golongan minuman berakohol yaitu golongan A; kadar etanol 1%-5% (bir), golongan B; kadar etanol 5%-20% (minuman anggur/wine) dan golongan C; kadar etanol 20%-45% (Whiskey, Vodca, TKW, Manson House, Johny Walker, Kamput).
Pada umumnya alkohol :

• Akan menghilangkan perasaan yang menghambat atau merintangi.
• Merasa lebih tegar berhubungan secara sosial (tidak menemui masalah).
• Merasa senang dan banyak tertawa.
• Menimbulkan kebingungan.
• Tidak mampu berjalan.

INHALANSIA atau SOLVEN
Adalah uap bahan yang mudah menguap yang dihirup. Contohnya aerosol, aica aibon, isi korek api gas, cairan untuk dry cleaning, tinner, uap bensin.Umumnya digunakan oleh anak di bawah umur atau golongan kurang mampu/anak jalanan. Penggunaan menahun toluen yang terdapat pada lem dapat menimbulkan kerusakan fungsi kecerdasan otak.

• Pada mulanya merasa sedikit terangsang.
• Dapat menghilangkan pengendalian diri atau fungsi hambatan.
• Bernafas menjadi lambat dan sulit.
• Tidak mampu membuat keputusan.
• Terlihat mabuk dan jalan sempoyongan.
• Mual, batuk dan bersin-bersin.
• Kehilangan nafsu makan.
• Halusinasi.
• Perilaku menjadi agresif/berani atau bahkan kekerasan.
• Bisa terjadi henti jantung (cardiac arrest).
• Pemakaian yang berlebihan dapat menyebabkan kerusakan syaraf otak menetap, keletihan otot, gangguan irama jantung, radang selaput mata, kerusakan hati dan ginjal dan gangguan pada darah dan sumsum tulang. Terjadi kemerahan yang menetap di sekitar hidung dan tenggorokan.
• Dapat terjadi kecelakaan yang menyebabkan kematian di antaranya karena jatuh, kebakar, tenggelam yang umumnya akibat intoksikasi/keracunan dan sering sendirian. bat intoksikasi/keracunan dan sering sendirian.

Efek narkotika tergantung kepada dosis pemakaian, cara pemakaian, pemakaian sebelumnya dan harapan pengguna. Selain kegunaan medis untuk mengobati nyeri, batuk dan diare akut, narkotika menghasilkan perasaan “lebih membaik” yang dikenal dengan eforia dengan mengurangi tekanan psikis. Efek ini dapat mengakibatkan ketergantungan. tanda tanda fisik, dapat dilihat dari tanda – tanda fisik si pengguna, seperti :
1. mata merah
2. mulut kering
3. bibir bewarna kecoklatan
4. perilakunya tidak wajar
5. bicaranya kacau
6. daya ingatannya menurun

Ada pun tanda – tanda dini anak yang telah menggunakan narkotik dapat dilihat dari beberapa hal antara lain :
1. anak menjadi pemurung dan penyendiri
2. wajah anak pucat dan kuyu
3. terdapat bau aneh yang tidak biasa di kamar anak
4. matanya berair dan tangannya gemetar
5. nafasnya tersengal dan susuh tidur
6. badannya lesu dan selalu gelisah
7. anak menjadi mudah tersinggung, marah, suka menantang orang tua

Lalu bagaimana mengetahui bahwa anggota keluarga jadi pecandu obat terlarang itu? Mardan Sadzali memberikan ciri-ciri yang mudah diketahui pada pecandu narkoba.

• Pecandu daun ganja : Cenderung lusuh, mata merah, kelopak mata mengattup terus, doyan makan karena perut merasa lapar terus dan suka tertawa jika terlibat pembicaraan lucu.

• Pecandu putauw : Sering menyendiri di tempat gelap sambil dengar musik, malas mandi karena kondisi badan selalu kedinginan, badan kurus, layu serta selalu apatis terhadap lawan jenis.

• Pecandu inex atau ekstasi : Suka keluar rumah, selalu riang jika mendengar musik house, wajah terlihat lelah, bibir suka pecah-pecah dan badan suka keringatan, sering minder setelah pengaruh inex hilang.

• Pecandu sabu-sabu : gampang gelisah dan serba salah melakukan apa saja, jarang mau menatap mata jika diajak bicara, mata sering jelalatan, karakternya dominan curiga, apalagi pada orang yang baru dikenal, badan berkeringat meski berada di dalam ruangan ber-AC, suka marah dan sensitive.

DAMPAK NARKOBA

DAMPAK NARKOBA

Bila narkoba digunakan secara terus menerus atau melebihi takaran yang telah ditentukan akan mengakibatkan ketergantungan. Kecanduan inilah yang akan mengakibatkan gangguan fisik dan psikologis, karena terjadinya kerusakan pada sistem syaraf pusat (SSP) dan organ-organ tubuh seperti jantung, paru-paru, hati dan ginjal.
Dampak penyalahgunaan narkoba pada seseorang sangat tergantung pada jenis narkoba yang dipakai, kepribadian pemakai dan situasi atau kondisi pemakai. Secara umum, dampak kecanduan narkoba dapat terlihat pada fisik, psikis maupun sosial seseorang.
Dampak Fisik:
1. Gangguan pada system syaraf (neurologis) seperti: kejang-kejang, halusinasi, gangguan kesadaran, kerusakan syaraf tepi
2. Gangguan pada jantung dan pembuluh darah (kardiovaskuler) seperti: infeksi akut otot jantung, gangguan peredaran darah
3. Gangguan pada kulit (dermatologis) seperti: penanahan (abses), alergi, eksim
4. Gangguan pada paru-paru (pulmoner) seperti: penekanan fungsi pernapasan, kesukaran bernafas, pengerasan jaringan paru-paru
5. Sering sakit kepala, mual-mual dan muntah, murus-murus, suhu tubuh meningkat, pengecilan hati dan sulit tidur
6. Dampak terhadap kesehatan reproduksi adalah gangguan padaendokrin, seperti: penurunan fungsi hormon reproduksi (estrogen, progesteron, testosteron), serta gangguan fungsi seksual
7. Dampak terhadap kesehatan reproduksi pada remaja perempuan antara lain perubahan periode menstruasi, ketidakteraturan menstruasi, dan amenorhoe (tidak haid)
8. Bagi pengguna narkoba melalui jarum suntik, khususnya pemakaian jarum suntik secara bergantian, risikonya adalah tertular penyakit seperti hepatitis B, C, dan HIV yang hingga saat ini belum ada obatnya
